Applicants must have 501(c)(3) status or have a partner 501(c)(3) organization that agrees to be the fiscal agent for the grant. Collaborative applications with local, state or federal agencies are encouraged.
Priority will be given to proposals that promote systemic change through strong, outcome-focused initiatives (including, but not limited to, community collaborations, environmental and programmatic interventions and policy advocacy) that target communities most burdened by a limited access to healthy food, limited access to physical activity, increased tobacco usage and high blood pressure rates.
Awards may be used for new initiatives that are unique and innovative, or for the expansion or continuation of already successful projects or initiatives.
To be considered, the Mohawk Valley Community Impact Grant online application and supplemental documentation must be received no later than Tuesday, March 31, 2020.
Eligible geographic region:Six counties in the Mohawk Valley region: Oneida, Herkimer, Otsego, Lewis, Madison and Hamilton.
